RFA track under the 'Make Wyoming Healthy Again' initiative, funding implementation of clinically-integrated care coordination to improve chronic disease management and health outcomes for rural and frontier populations. Supports innovative, evidence-based, and tech-enabled solutions targeted at high-risk groups to reduce barriers to care and promote population health.
Eligible applicants are defined in the Wyoming RHTP Submittable portal and must follow all requirements in the current RFA card and accompanying guidance. Applicants must generally be Wyoming-based organizations capable of implementing clinically-integrated care coordination for chronic disease in rural/frontier communities as defined by the program guidance.
