Requirements Checklist

• Mandatory submission requirements: (1) Exhibit A: Applicant Forms and Certifications (including Eligibility Checklist, Organization Info, Certifications and Assurances, signed by an authorized representative); (2) Exhibit B: Application Questions (complete responses covering organizational background, rural service, implementation, sustainability, budget narrative, health equity, workforce commitment); (3) Exhibit D: Budget Proposal Template (Excel, detailed for each performance period and category, with justification—max $500,000 per period); (4) Licensure documentation; (5) Evidence of rural client service (showing HRSA/OFM/CMS rural designation compliance); (6) Employment evidence for clinicians meeting WA DOH and federal OTP criteria. All application materials must follow RFA-provided numbering/headings and be submitted in the specified formats; no zipped files. Demonstrate OTP OUD service capacity, affirm five-year workforce obligation, comply with funding rules, and document federally approved indirect cost rates if claiming indirect.

No zipped files. Authorized representative signature is required. Late or incomplete submissions are disqualified unless substantiated HCA email outage occurs.
