Virginia RHTP: Provider Interoperability — Where Connection Meets Care

Eligibility

Applicants must be legally organized entities in good standing (including nonprofits, governmental units, Tribal entities, hospitals/health systems, independent practices, free/hybrid/charitable clinics, federally qualified health centers (FQHCs), rural health clinics (RHCs), educational institutions, or health care associations) capable of receiving federal subawards. Applicants must hold active SAM.gov registration with a current Unique Entity Identifier (UEI) and no active exclusions or debarments. Eligible applicants must be physically located in or provide direct services to rural Virginia communities as defined by the Federal Office of Rural Health Policy (FORHP) and the Virginia RHTP (see Appendix A of the RFA for designated localities). State agencies may apply when permitted by the RFA. Applicants currently excluded from federal funding are ineligible. When relevant, formalized partnerships must be documented with signed letters of support or memoranda of understanding specifying roles and effort levels for each participating organization.

Requirements Checklist

• Applications must be submitted using the designated online Application Submission Form by the posted deadline. Required materials include: an application cover page (entered into the online form); a project narrative conforming to sectioning and page/format limits (Sections A–H, 11-point Arial, double-spaced, 1-inch margins); a detailed budget and budget narrative using the VHCF template; resumes for key programmatic staff; signed letters of support or memoranda of understanding for any formal partners; most recent audited financial statements or Federal Single Audit; documentation of active SAM.gov registration and debarment certification (screenshot acceptable); and adverse action disclosures. The narrative must address the organizational overview, need/project design, implementation readiness, work plan and staff, stakeholder engagement, performance metrics, evaluation, sustainability, and prior grant experience. Applications not implementation-ready or noncompliant with requirements will not be reviewed.
