Virginia RHTP: CareIQ Remote Patient Monitoring Initiative

Summary

The Virginia Hospital & Healthcare Association Foundation (VHHAF) invites applications for the CareIQ Remote Patient Monitoring (RPM) Initiative under Virginia’s Rural Health Transformation Program (RHTP), supported by CMS/HHS. This RFA seeks licensed hospitals, health systems, or academic medical centers to design, implement, or expand hospital-based RPM programs serving rural communities, as defined by the Federal Office of Rural Health Policy (FORHP). The initiative targets technology-enabled chronic disease management and post-acute follow-up, aiming to reduce avoidable hospitalizations and readmissions, strengthen care coordination, and improve patient engagement and outcomes through RPM devices, EHR integration, and clinical workflow redesign. $14.3 million is available for Budget Period 1 (10/30/2026–9/30/2027), with 2–3 awards anticipated; subsequent funding may be available for up to four additional years based on demonstrated measurable outcomes. No cost sharing or match is required. Applicants must provide detailed implementation plans, sustainability plans, and comply with federal subrecipient, reporting, and risk assessment requirements.

Eligibility

Licensed hospitals, health systems, or academic medical centers that are physically located in or provide direct services to rural Virginia communities as defined under Virginia's RHTP, using the Federal Office of Rural Health Policy (FORHP) definition and current eligibility requirements. Applicants must possess active SAM.gov registration with a current UEI, have no active exclusions, debarments, or suspensions, and must demonstrate capacity to manage federal funds per 2 CFR Part 200. Partnering with technology vendors or care management organizations is permitted but the applicant must serve as lead. Entities with federal exclusions, debarments, incomplete/inaccurate applications, or non-compliant financial/audit histories are ineligible.
