Utah RHTP Monitoring and Evaluation Contract (RFSQ BB27-100)

Summary

RFSQ seeking a contractor to provide statewide monitoring and evaluation services for Utah's Rural Health Transformation Program (RHTP) -- a 5-year, CMS-funded initiative ($195.7M Year 1 for Utah) covering 25 rural/frontier counties across seven initiatives (PATH, RISE, SHIFT, FAST, LIFT, SUPPORT, LINCS). Vendor will assess program outcomes, evaluate the seven initiatives and pilot activities, and monitor performance across the five-year program.

Eligibility

Eligible vendors must be recognized legal entities (for-profit, non-profit, or government), able to prove legal operational status and formally attest to their eligibility and intent to register as an active vendor with Utah State Purchasing within 14 business days of Notice of Intent to Award; must comply with all federal CMS funding requirements, including 10% administrative cap, and be responsive under Utah Procurement Code 63G-6a-120 and 902. Vendors may not be suspended, debarred, subject to tax liens, or have unresolved conflicts impacting responsiveness.

Requirements Checklist

• Applicants must submit statements of qualifications outlining expertise in monitoring and evaluation of large-scale, federally funded health programs; demonstrate capacity for baseline, midline, and endline evaluation; provide a multi-year project plan and itemized budget; comply with the CMS 10% administrative cap and Stevens Amendment; respond to evaluation criteria as set forth under Utah Procurement Code 63G-6a Part 15. Include past performance, proposed key personnel and evidence of ability to meet reporting, federal compliance, and insurance requirements. No pricing is to be included with the initial submission.
