SHIFT 3.2: Rural Health Provider Networks

Summary

Utah Rural Health Transformation Program grant to establish or expand integrated rural health provider networks (e.g. rural health clinics, pediatric trauma, or maternal health networks). Awardees build a comprehensive resource directory of physical and behavioral healthcare services across rural Utah. Funding type: grant agreement. Up to $2,500,000 available for Year 1 (approximately 6 awards anticipated; individual Year 1 awards range $250,000-$1,250,000 depending on project scale and network complexity); up to $18,500,000 over a 5-year period for the full agreement, contingent on availability of funds. Eligible applicants: Utah-based strategic partners with extensive expertise in rural health systems, existing relationships with rural hospitals, clinics, and healthcare organizations, and demonstrated technical capacity to develop a comprehensive resource directory tailored to rural Utah.

Eligibility

Eligible applicants must be Utah-based entities with substantial experience in rural health care systems, including, but not limited to, rural hospitals, health clinics, community-based organizations, and other strategic partners with technical capacity and established relationships with rural providers. Applicants must demonstrate prior success in leading health network development and in coordinating physical and behavioral health resources in underserved rural areas across Utah.

Requirements Checklist

• Applicants must submit a full application package including: (1) Cover sheet, (2) Proof of eligibility, (3) Proposed project narrative addressing required elements in the RFGA, (4) Detailed budget and budget narrative, (5) Letters of commitment from network partners, if applicable, (6) Evidence of prior system integration work. Applications must follow formatting and submission instructions listed in Section 6 of the RFGA. Late or incomplete submissions will not be reviewed.
