The Texas Health and Human Services Commission (HHSC) is offering the Rural Hospital Improvement Grant (RFA No. HHS0015677) to provide time-limited financial support for qualified rural hospitals in Texas. The program aims to improve the financial stability of rural hospitals through hospital improvement activities. Up to 33 hospitals may each receive up to $350,000 from a total grant pool of $11,550,000 for FY2025. Funding is time-limited and tied to state legislative appropriations. No local cost share or match is required. The Project Period runs from the effective date of the Grant Agreement through August 31, 2026. Applicants must submit a Hospital Financial Stability Improvement Plan and comply with quarterly reporting requirements. Eligible hospitals must be classified as rural by population thresholds or Medicare designation as defined in the RFA.
Eligible applicants are rural hospitals in Texas, defined as hospitals that: (A) are located in a Texas county with 68,750 or fewer persons according to the 2020 U.S. Census; (B) are designated by Medicare as a Critical Access Hospital (CAH), Sole Community Hospital (SCH), or Rural Referral Center (RRC) not located in a Metropolitan Statistical Area; or (C) have 100 or fewer beds, are designated by Medicare as a CAH, SCH, or RRC, and are located in a Metropolitan Statistical Area.
