SHPDA seeks a qualified contractor to establish, operationalize, and support a statewide Clinically Integrated Network (CIN) and Managed Services Organization (MSO) shared-services model for eligible independent and community-based primary care providers and organizations. Services may include provider recruitment/onboarding, EHR/EMR and health information technology modernization, interoperability and analytics, care management and practice transformation, accountable-care and multi-payer readiness, financial operations, sustainability planning, and, if authorized, administration of SHPDA-approved Participation Support. Funded through CMS's Rural Health Transformation Program.
Eligible applicants are independent or community-based primary care providers and organizations within the State of Hawaii.
