Rural Health Transformation School-Based Health Center (RT27)

Summary

The Ohio Department of Health, Bureau of Maternal, Child, & Family Health invites competitive applications from local or public non-profit healthcare agencies for projects to plan, establish, expand, or sustain school-based health centers (SBHCs) in designated rural Ohio counties. Supported activities include SBHC business planning, site renovations, equipment purchase, outreach, staffing, evaluation, and participation in ODH technical assistance. SBHCs must provide accessible medical, mental, dental, and vision care directly in schools for rural populations. Projects may target new sites, start-up operations, or the expansion/sustainability of existing SBHCs.

Eligibility

Eligible applicants are local or public non-profit healthcare agencies seeking to establish, expand, or sustain a school-based health center (SBHC) located in, and serving students from, the following eligible rural Ohio counties: Adams, Allen, Ashland, Ashtabula, Athens, Auglaize, Belmont, Brown, Carroll, Champaign, Clark, Clinton, Columbiana, Coshocton, Crawford, Darke, Defiance, Erie, Fairfield, Fayette, Fulton, Gallia, Geauga, Greene, Guernsey, Hancock, Hardin, Harrison, Henry, Highland, Hocking, Holmes, Huron, Jackson, Jefferson, Knox, Lawrence, Licking, Logan, Madison, Marion, Medina, Meigs, Mercer, Miami, Monroe, Morgan, Morrow, Muskingum, Noble, Ottawa, Paulding, Perry, Pickaway, Pike, Preble, Putnam, Richland, Ross, Sandusky, Scioto, Seneca, Shelby, Tuscarawas, Union, Van Wert, Vinton, Warren, Washington, Wayne, Williams, Wyandot.

Requirements Checklist

• Applicants must submit both draft and final SBHC business plans. Minimum 8 advisory committee meetings per project period are required. Quarterly data reports must be submitted. Participation in ODH meetings and technical assistance sessions is mandatory. Allowable activities include staffing, equipment procurement, renovation, outreach, and evaluation.
