This opportunity, announced May 8, 2026, anticipates approximately 50 awards for projects that may include: expanding access to essential health and behavioral health services, enhancing workforce development, strengthening healthcare infrastructure, deploying telehealth solutions, procuring equipment, conducting limited facility renovations (with costs capped at 20% of a project's total budget), planning, training, and related capacity-building activities. Eligible applicants must directly benefit rural or tribal North Dakota residents. Administrative expenses are capped at 10% of the budget, provider payments at 15%, and renovation/alteration costs at 20%. Unallowable costs include construction, meals/food, and supplanting existing funding. All activities and costs must comply with relevant ND HHS and federal requirements (including 2 CFR Part 200 and 300). Rural/urban eligibility definitions and application requirements are clarified in the primary NOFO, with urban entities eligible only if their application targets rural beneficiaries or rural use of funds. Applicants must articulate measurable and sustainable health improvements, especially for Medicaid members, and applications must be submitted via the Qualtrics portal using all required templates by the published deadline.
Eligible applicants include organizations based in or serving rural or tribal regions of North Dakota. Urban entities (located in Grand Forks, Fargo, West Fargo, or Bismarck) are eligible only if at least 50% of project beneficiaries are rural ND citizens or all funds are used for rural ND residents. Eligible applicants: hospitals, clinics, home care providers, critical access hospitals, long-term care facilities, behavioral health providers, tribes and tribal health organizations, EMS providers, other health care providers, transportation providers, local public health, nonprofit organizations, educational institutions/systems, community organizations, faith-based organizations, parks and recreation, municipalities, and civic groups. For-profit providers are eligible as subrecipients/subcontractors only, not direct awardees.
