The program aims to strengthen rural health care delivery through minor physical renovations and refurbishments, enabling network and workforce enhancements, technology advancement only as part of facility upgrades, and improved access for Vermonters. Eligible projects support service realignment, shared operations, and integrated care delivery envisioned under Vermont’s regionalization strategy. Upgrades must comply with federal and state requirements, including safety standards, building codes (ADA Accessibility Standards), and reporting. Year 1 grants support expenditures within the grant period. Funding restrictions exclude pre-award costs, construction/expansion, demolition, cosmetic upgrades that materially increase property value, and supplanting existing funding.
Eligible applicants are hospitals and nonhospital providers physically located in Vermont, including but not limited to primary care and specialty care practices, home health and hospice agencies, skilled nursing and long-term care providers, adult day centers, mental health and substance use disorder providers, and community retail pharmacies. Providers located in rural ZIP codes (Appendix B Table 1) are eligible. Federally Qualified Health Centers (FQHCs), home health and hospice agencies, and community mental health centers in non-rural/urban ZIP codes (Appendix B Table 2) are also eligible. Applicants must have full SAM registration and a valid Unique Entity Identifier (UEI) to be eligible.
