Remote Patient Monitoring: Adult Chronic Disease and Perinatal - RPM Provider(s)

Summary

Provide a scalable statewide Remote Patient Monitoring service that supports adults with hypertension, heart failure, and other high-risk chronic conditions, as well as pregnant and postpartum individuals requiring maternal health monitoring. Services will improve chronic disease management and maternal health outcomes through ongoing monitoring, patient engagement, self-management support, timely intervention, care coordination, expanded access to home-based monitoring services, and delivery of a standardized statewide RPM model.

Eligibility

Open to organizations capable of delivering Remote Patient Monitoring programs in Maine, including health systems, provider networks, and technology-enabled care delivery entities. Focus is on partners serving rural, high-risk, and underserved populations.

Requirements Checklist

• Applicants must demonstrate capacity for statewide service delivery for adult chronic disease and perinatal populations, including implementation of telehealth RPM technology, patient engagement workflows, and data integration reporting. Proposals should address scalability, care coordination, outreach to rural and underserved populations, and quality improvement measures.
