Pennsylvania Rural Health Transformation Plan Rapid Response Stabilization Payments — Round 2

Summary

the statewide cap is met. Initial regional caps limit awards to two payments per RHTP region until all regions have two payments; then the cap lifts. Required attachments include: Entity Location List (if multi-location), license/certificate, documentation of provider/facility status, Budget Narrative and Budget Worksheet (templates), third-party quote/estimate, FFATA data sheet for requests >$30,000, and Funding Disclosure Table. All costs must be for eligible supplies and equipment that stabilize or enhance rural healthcare (no EHRs, ambulances, or ongoing projects requiring departmental monitoring), expended by July 31, 2027, and documented within 30 days of incurrence. Entities must retain audit records until February 27, 2031.

Eligibility

Eligible applicants are hospitals licensed as general acute care by the Pennsylvania Department of Health, health care providers as defined in 42 U.S.C. §1397ee(h)(9), or rural health facilities as defined in 42 U.S.C. §1397ee(h)(3)(D), operating in Pennsylvania since at least December 29, 2025. Eligible locations include rural counties (class 4–8) or HRSA-defined rural census tracts in counties of the second class A or third class. Applicants that have received Round 1 payments (May 1–June 1, 2026) are ineligible.

Requirements Checklist

• Applicants must complete the Rapid Response Stabilization Round 2 Certification via webform (live August 17, 2026 at 8:00 AM) or submit the PDF form and required attachments by email to RA-HHRRLHLTHTRNSPLAN@pa.gov. Attachments required: Entity Location List (if multi-location), license or certificate of compliance, documentation confirming provider/facility status (if not licensed), proof of PA registration/incorporation, Budget Narrative and Budget Worksheet (templates), third-party quote/estimate, FFATA Data Sheet (> $30,000 requests), Funding Disclosure Table (if applicable). All payment requests must be for supplies/equipment that stabilize or enhance rural healthcare, relate to at least one RHTP initiative, and not duplicate prior public funding or supplant other federal/state funding.
