Pennsylvania lines up FQHC & FQHC Look-Alikes - Modernizing Healthcare Technology and Enabling Interoperability opportunity

Summary

The program is open to Medicaid-enrolled FQHCs and look-alikes operating in Pennsylvania with at least one physical location, in full operation since at least December 29, 2025, and not at risk of closure. One payment per entity is permitted. Documentation requirements and monitoring/audit provisions apply; funds cannot supplant other federal/state awards.

Eligibility

Applicant must be (1) a Medicaid-enrolled provider that is either a Federally Qualified Health Center (FQHC) meeting Section 330 of the Public Health Service Act (42 U.S.C. § 254b), or an FQHC Look-Alike that meets federal qualifications under Section 330; (2) currently operating with at least one physical location in Pennsylvania that has been in operation since at least December 29, 2025; (3) located in one or more Pennsylvania RHTP eligible rural counties classes 2A-8 or HRSA-defined rural census tracts in classes 2A and 3; (4) in good standing and not at risk of closure on date of receipt of funds; (5) provides services to residents of eligible rural counties; (6) enrolled provider in Pennsylvania’s Medical Assistance Program (Medicaid).

Requirements Checklist

• Funds must be used solely for purchasing and implementing a Certified Electronic Health Record Technology (CEHRT) system in accordance with 45 CFR § 170.315 and for becoming a participant and fully onboarding with one of Pennsylvania's five certified Health Information Organizations (HIOs), part of the Pennsylvania Patient and Provider Network (P3N). Full onboarding to a P3N HIO must occur within 90 days of CEHRT implementation. All expenditures must be incurred by June 30, 2027. Program payment recipients must submit documentation supporting use of funds within 30 days of incurring costs (e.g., itemized receipts or invoices) and maintain audit records through February 27, 2031. Funds may cover payments to third-party consultants guiding CEHRT implementation and HIO onboarding. Funds cannot be used for costs already incurred, ongoing licensing, maintenance, upgrades, expansions, interoperability enhancements, indirect/administrative costs, or to supplant other federal/state funding. Any unused funds as of June 30, 2027 must be returned by July 31, 2027.
