The Nebraska Department of Health and Human Services, Chronic Disease Prevention Program, invited proposals to expand chronic disease management through community-based education, care navigation, and self-management support — helping people with conditions like diabetes, cardiovascular disease, and asthma connect with resources and navigate the health system. Funded under Nebraska's RHTP Initiative 4.4a. Awards were capped at $400,000 per applicant; 24 organizations were selected for award (announced May 6, 2026), totaling $6,594,460.94.
Eligible applicants include rural clinics, critical access hospitals, frontier hospitals, tribal health facilities, Federally Qualified Health Centers (FQHCs), Local Public Health Departments, behavioral health organizations, and consortia serving primarily rural or high social vulnerability index (SVI) populations in Nebraska. Collaborative applications and organizations with established care navigation or chronic disease education programs must demonstrate expansion, non-duplication, and service to targeted populations.
