NC ROOTS — Trillium: LOCATe Implementation (Regions 2 & 5)

Requirements Checklist

• Applicants must:
• Implement the CDC LOCATe tool across all eligible birthing hospitals in the designated region (Region 2 or 5).
• Train facility teams, coordinate survey completion, and collect, validate, and analyze assessment data.
• Incorporate community and stakeholder input, including tribal engagement, to validate findings and recommendations.
• Submit specified project deliverables (work plan, training curriculum, assessment reports, etc.) by required deadlines.
• Operate in accordance with federal reimbursement policies (i.e., upfront payment of costs, documented invoicing).
• Maintain compliance with all federal (including 2 C.F.R. Part 200, HIPAA, and Stevens Amendment) and state laws, regulations, and reporting requirements.
• Disclose and mitigate any conflicts of interest, and ensure all lower-tier subrecipients/subcontractors are approved and compliant.

Submission Instructions

Applications must be submitted electronically to the designated region-specific email addresses (Region2NCRootsHub@TrilliumNC.org for Region 2, Region5NCRootsHub@TrilliumNC.org for Region 5) by August 31, 2026, 4:00 PM EDT. Late, faxed, mailed, or hand-delivered applications will not be accepted.
