This opportunity funds the implementation of the CDC's Levels of Care Assessment Tool (LOCATe) across all birthing hospitals in Region 2 (13 counties) and Region 5 (15 counties) of North Carolina. Supported by the CMS-funded North Carolina Rural Health Transformation Program (NCRHTP), the project aims to establish a standardized baseline of hospital-level maternal and neonatal care capacity, identify regional service gaps, guide facility improvements, and address perinatal health disparities, especially in rural, high-need, and underserved areas. Applicants must engage all eligible birthing hospitals, coordinate survey completion, analyze and report assessment findings, and ensure culturally responsive stakeholder and tribal engagement. The anticipated award per region is $50,000–$100,000 for Year 1, contingent on federal appropriations. Awards will be made via Trillium's standard Subaward Agreement, subject to all federal and state requirements, including compliance with 2 C.F.R. Part 200, data security, and reporting obligations. Multiple awards may be made based on quality, regional fit, and available funding.
Eligible applicants are legally organized entities (nonprofits, health systems, local governments, academic institutions, etc.) authorized to conduct business in North Carolina and possessing a Unique Entity Identifier (UEI) from SAM.gov. Applicants must serve, or propose to serve, populations located within Trillium Health Resources Regions 2 or 5, demonstrate capacity for maternal/neonatal hospital assessment and community engagement, have sufficient financial resources to operate on a reimbursement basis, and not be debarred or suspended from federal/state funding. Experience working with underserved, rural, or low-income populations and culturally responsive engagement is required. Organizations must be able to complete all required deliverables and adhere to all applicable compliance policies.
