Trillium Health Resources, as NC ROOTS Hub Lead for Regions 2 and 5 under the NC Rural Health Transformation Program (NCRHTP), is soliciting applications to implement the 'I Gave Birth' postpartum support program targeting maternal health improvement in rural and urban-rural counties. The initiative focuses on provider training, patient outreach, structured follow-up, stakeholder engagement, and strategies to reduce maternal health disparities through hospital and community partnerships. Successful applicants will collaborate with birthing facilities and NCDHHS, develop training curricula, collect program and enrollment data, engage NC tribal communities, and build sustainable programming in alignment with CMS-funded rural health goals. Awards anticipated at $50,000–$100,000 per region for GAP Year 1; reimbursement basis; activities subject to federal requirements under CMS and NC DHHS. Multiple or no awards possible.
Applicants must be legally organized entities such as corporations, nonprofits, academic institutions, governmental entities, or sole proprietorships authorized to conduct business in North Carolina, with a Unique Entity Identifier (UEI) from SAM.gov. Eligible organizations must demonstrate financial capacity, relevant expertise in postpartum support/maternal health, community engagement experience, and not be debarred/suspended/excluded from government contracts. Preference given to those with direct rural/region experience, serving high-need and underserved populations, familiarity with NC Medicaid and I Gave Birth models, continuous operation >2 years, and NC HUB/MWBE status.
