Live Well, Together: Food Is Medicine Initiative — Request For Applications

Summary

This RFA solicits applications from eligible nonprofit organizations with statewide reach to coordinate and administer Virginia’s Food Is Medicine pilot program for rural regions. The initiative aims to improve access to nutritious food, deliver nutrition education, and support chronic disease self-management for rural Virginians experiencing food insecurity and at risk of chronic disease. Selected grantees will implement evidence-based models such as medically tailored meals, produce prescriptions, and nutrition education, in partnership with healthcare providers and community food access organizations, to demonstrate sustainable approaches and improved health outcomes.

Eligibility

Eligible applicants must be nonprofit organizations with a demonstrated statewide presence and the capacity to coordinate and administer a Food is Medicine program across multiple rural regions of Virginia as defined under Virginia’s RHTP, using the federally required FORHP definition as the baseline and the most current eligibility posted on the RHTP website. Applicants must provide evidence of successful partnerships with health systems, rural healthcare providers, community-based organizations, food access organizations, and other stakeholders relevant to the delivery of food and nutrition interventions. Applicants must obtain and maintain a valid Unique Entity Identifier (UEI) and have no active exclusions, suspensions, or debarments preventing federal funding. An active SAM.gov registration is not required.
