Kentucky is soliciting Kentucky-based training organizations to develop new specialized Community Health Worker (CHW) certificates in four priority areas: Healthy Lifestyle Management, Perinatal and Infant Health, Mental and Behavioral Health, and Dental/Oral Health. Maximum award is $200,000 per certificate, up to $800,000 total for all four, paid via milestone-based payments. Training will be delivered free through Kentucky's TRAIN platform. Applications due August 5, 2026 at 5:00 PM ET; anticipated funding period begins October 7, 2026. Part of Kentucky's Rural Health Transformation Plan, funded by CMS.
Eligible applicants are Kentucky-based non-profit training organizations, including approved CHW training organizations, public or private colleges and universities, community/technical colleges, local health departments, government agencies, Area Health Education Centers (AHEC), community-based workforce organizations, professional healthcare associations, professional training organizations. Applicants must: be based in Kentucky, be a non-profit organization, demonstrate capacity to educate and train CHWs per KOCHW standards, be in good standing with the Commonwealth of Kentucky Secretary of State, have experience designing and executing public health training, agree to deliver curriculum via the TRAIN platform, include CHWs in curriculum development, provide training free of charge, identify a single lead organization, clearly identify and budget for any subcontracting, and demonstrate support for rural Kentucky communities.
