Crisis to Care: Establish or Expand Community Paramedicine Program

Summary

The Rural Health Transformation Program (RHTP) Community Paramedicine RFA provides up to $20 million in competitive awards for Kentucky EMS agencies to establish or expand community paramedicine services in rural areas. This opportunity prioritizes licensed EMS agencies (public or nonprofit) seeking to implement new or enhanced care models, expand workforce, invest in technology, and develop partnerships, particularly with behavioral health providers. Supported activities include mobile crisis response, care continuity, training, and integration with the broader healthcare system. Funding is 100% provided by CMS/HHS and must be used during the specified performance period. Grants are intended for rural, KBEMS-licensed agencies launching or scaling sustainable models that address local needs and demonstrate capacity for long-term impact.

Eligibility

Eligible applicants are public or nonprofit EMS agencies licensed by the Kentucky Board of EMS (KBEMS) as Class Ia or Class Ib and operating in Kentucky. Agencies must provide 911 advanced life support (ALS) services within the Commonwealth and maintain good standing for the award duration. Priority is given to rural-serving agencies and those demonstrating readiness to partner with behavioral health organizations.

Requirements Checklist

• Applicants must complete and submit Attachment A (Application Form, maximum 25 pages excluding attachments), including agency details, personnel/staffing, service area, description of current and proposed paramedicine services, partnerships (especially in behavioral health), implementation timeline, detailed FY27 budget using Attachment B, and letters of support (such as from a medical director). Submissions must document KBEMS licensure, describe technology utilization, crisis response programming, and include an attestation of compliance. Late or incomplete submissions will not be considered.
