Illinois RHTP: EMS and Mobile Healthcare — Mobile Health Infrastructure Funds

Funds mobile health infrastructure for primary care and behavioral health providers. Target: release funding opportunities/select providers Q3-Q4 2026.

Eligibility

Eligible applicants include Illinois-based primary care and behavioral health providers participating in the Rural Health Transformation Program. Funding is distributed via sub-awards to the Illinois Primary Health Care Association (IPHCA) and Community Behavioral Healthcare Association (CBHA) for further allocation to qualifying member organizations.

Requirements Checklist

• Applicants must be members of IPHCA or CBHA and operate as licensed primary care or behavioral health providers in Illinois' rural service areas. Funded activities must support mobile health infrastructure upgrades or service expansion and comply with all reporting and expenditure requirements established by HFS and CMS. All funds must be obligated before October 30, 2026 for Year 1 activities.

Submission Instructions

Applicants will be invited and submit materials via instructions disseminated by IPHCA or CBHA as sub-award intermediaries. There is no standalone public RFP or application for this funding stream.
