Eligibility

Eligible entities are pre-selected rural Healthcare Transformation Collaborators as subrecipients: Carle Health, Egyptian Health Department, OSF Healthcare, SIHF Healthcare. No open competitive application process; eligibility is determined by named sub-awardee status in the CMS-approved RHTP implementation plan.

Requirements Checklist

• Subrecipients must implement collaborative healthcare transformation activities as outlined in the RHTP application and budget amendment. Awarded funds must be obligated before the end of the budget period, specifically by 10/30/2026 for Year 1. Reporting and compliance are governed by CMS and HFS standards for subrecipient management.
