Supports rural primary care transformation projects in New Hampshire across FQHCs, RHCs, Critical Access Hospitals, and other rural clinics through team-based, technology-enabled, value-driven, and sustainable models. Provides direct funds for three program stages: Project Development, Preparatory Investments, and Program Ready implementation projects. Higher awards are available for regional, multi-organization partnerships demonstrating shared infrastructure, service delivery, or risk. Proposals must address all four Rural Primary Care Focus Areas: 1) Innovative care models, 2) Technology adoption with ROI, 3) Transition to value-based care, and 4) Financial sustainability and operational efficiency. Priority given to organizations serving Medicaid, with clear sliding fee policies, and those demonstrating strong partnerships for integrated, scalable change.
Eligible applicants: Organizations that deliver primary care, including Federally Qualified Health Centers (FQHCs), FQHC Look-Alikes, Rural Health Clinics (provider-based or independent), physician or nurse practitioner-led rural primary care practices, and hospitals with a primary care clinic or rural health clinic. Organizations partnering with primary care practices to transform delivery are also eligible. Applicants must meet rural eligibility through one of two pathways: Path A – location in a HRSA-defined rural area, Path B – holding a federal rural designation (RHC or FQHC). The lead applicant must be an eligible organization; regional partnerships are encouraged, and partners may include EMS, oral and behavioral health, home health agencies, and other community entities. Exclusions: standalone specialty practices not proposing integration with primary care; organizations that contributed to the design of this RFA (per 2 CFR §200.318(c)); activities already funded by GO-NORTH or GO-NORTH Hub; unresolved legal proceedings with government entities.
