Requirements Checklist

• Submit an online application, addressing all four required Rural Primary Care Focus Areas and identifying an appropriate program stage (Project Development, Preparatory Investments, Program Ready). Applications must include a workplan, detailed project budget (with written justifications for each budget line), Letters of Commitment or fully executed MOUs from all funded regional partners (if applicable), sliding fee/financial assistance policy, payer mix documentation, and proof of Medicaid beneficiary service. Only one application per lead entity is allowed. All personnel costs, equipment over $10,000, and operational expenditures must comply with 2 CFR Part 200. Initial application deadline applies for cohort review; rolling submissions accepted only while funds remain.

• Certificate of Good Standing from NH Secretary of State
• Unique Entity Identifier (UEI) and NH State Business ID
