Funds the establishment of a new primary care medical clinic in an underserved rural Georgia county through accredited university medical schools. Priority specialties are Family Medicine, Internal Medicine, Pediatrics, OB/GYN, or General Surgery. Grants may be used for site acquisition, renovation, equipment, staff salaries, and community marketing. Clinics must provide clinical rotations for medical students and may partner with local hospitals/practices to establish new sites (not expansions). Applicants must demonstrate ongoing leadership support, a dedicated project manager, and sustainability beyond the grant period. Required value-added services may include care coordination, behavioral health, or other community health supports. Award is up to $800,000 for a one-year project period, subject to available appropriations, with up to four awards.
Eligibility
Eligible applicants are Georgia universities with an accredited medical school. Proposed practice sites must be in counties designated as rural under the Rural Hospital Organization Assistance Act of 2017, or a county with a critical access hospital. Applicants must accept commercial insurance, Medicare, and Medicaid, and provide student clinical rotation site opportunities to Georgia medical students. Applicants must be in good standing with DCH/SORH (no sanctions or suspensions) and may partner with hospitals or practices, but funding may only be used to establish a new site, not expand existing practices.
