The Minnesota Department of Health 2027 Community Clinic Grant Program supports eligible clinics serving low-income and rural populations through preventive, medical, dental, and mental health primary care services. The competitive RFP provides funding for projects that improve access to primary care services for uninsured and underinsured Minnesota residents, with preference for those serving uninsured populations. Eligible clinics may be nonprofit, government-operated, tribal, or consortia, and must utilize a sliding fee scale or equivalent policy to ensure no person is denied services due to inability to pay. Grant funds support staff, outreach, equipment, and care delivery enhancements. Applications are due October 30, 2026, and awards will be made to clinics across metropolitan and rural areas to promote geographic equity.
Eligible applicants include nonprofit clinics, government entities operating clinics, Indian Health Services units or Indian tribal governments operating clinics, and consortia of these entities. Clinics must provide preventative, medical, dental, or mental health primary care services to low-income or rural populations, utilize a sliding fee scale or similar patient payment policy to ensure access regardless of ability to pay, and may be stationary or mobile. Clinics focused solely on urgent or emergent care are not eligible.
