the Rural Health Transformation Plan (the Plan) and is funded under the Cooperative Agreement with the Federal Rural Health Transformation Program (RHT Program). The amended NOFO lists $5.7 million for Program Year 1 and anticipates 25–30 connectivity/equipment awards averaging $70,000 and 25–30 education awards averaging $110,000. Awards will be made to subrecipients, subject to Federal and State requirements, on a reimbursement basis except where advance funding is demonstrated to be necessary and is limited as a percentage of the total award. Funds must be obligated by October 30, 2026 and project costs incurred by July 31, 2027. Indirect and administrative costs may not exceed 5%. All funded projects must provide meaningful benefit to rural Mississippi as defined in the NOFO, and subrecipients are required to participate in the Statewide Health Information Exchange.
Eligible applicants are Mississippi-located hospitals, physician practices (including solo practitioners), rural health clinics, federally qualified health centers, local health departments, Native American Sovereign Tribe healthcare facilities, certified community behavioral health clinics, substance use disorder facilities, dental care services, and licensed long-term care facilities located in rural areas or serving rural populations. Organizations do not need to be physically located in a rural area, but proposed activities must meaningfully benefit rural Mississippi. The NOFO defines rural as counties with fewer than 50,000 residents, fewer than 500 people per square mile, or municipalities with populations under 15,000. Awardees must possess or obtain a UEI, FEIN, and Mississippi MAGIC Vendor Number and participate in the statewide Health Information Exchange.
