The 2026-2028 FQHC Subsidy Grant Program provides funding to eligible Federally Qualified Health Centers (FQHCs) in Minnesota based on a statutory formula, proportionate to the amount of discounts awarded to patients in the prior calendar year. Eligible organizations must submit an annual letter of intent and complete a detailed application, with awards allocated to support discounted services to underserved populations. The goal is to help FQHCs offset financial losses from providing discounted healthcare and ensure access to care for vulnerable Minnesotans. Federally Qualified Health Centers (FQHCs) in Minnesota are eligible. Only discounts provided within Minnesota are counted for multistate organizations.
