Julota is a community care coordination and data infrastructure platform designed to help rural communities, EMS agencies, public health departments, behavioral health providers, hospitals, and community-based organizations work together around shared clients, shared referrals, and shared outcomes.
Many rural health programs are asked to improve access, reduce avoidable emergency department use, support high-risk patients, address behavioral health and substance use needs, and prove impact across multiple partner agencies. Julota helps communities do that by creating a secure operational layer where cross-agency teams can coordinate care, manage referrals, document interventions, close referral loops, and generate the reporting needed to demonstrate program value.
