Rural In-Home Monitoring and Alerting Program: An in-home assistive monitoring model designed to help recently discharged cardiac and medically complex patients remain safe, stable, and supported in their homes and communities.

Hospitals use the Empowered-Health platform for continuously monitoring patients discharged to reduce 30-day readmissions and normalize E.D. utilization rates, flattening spikes and improving schedule predictability. Documented patient progress data help them prove improvement in quality metrics.

How it works

Our remote health monitoring platform can be used with your nurses, or it can be augmented with ours. We provide training on how to use the intuitive clinician dashboard and report on patient health trends. We can optionally connect to your EMR to ingest patient C-CDAs for health background on the patient and put back enriched data into the EHR contained in your EMR system. We can also connect to a nationwide network called CareQuality and QHINS managed by TEFCA. This is a lightweight system giving clinicians just what they need to know to do a better job triaging patients and getting to the root cause of the condition.

Standard package includes

We offer three packages to choose from or upgrade to:

1. Base solution with cellular-enabled devices (scale, BP cuff)

2. Mid-tier solution with Apple or Garmin Watch, BP cuff, scale, CGM for pre-diabetes or diabetes, and digital thermometer / BYOD

3. Premium solution with Garmin Venu or Apple watch, BP cuff, advanced body scan scale, iPhone preloaded and pre-paired with devices, plus optional devices based on need (peak flow meter, Dexcom CGM - Stelo or G7)

Optional add-ons

Health Exchange Interoperability Package (HXI): Full bi-directional EMR connectivity plus access to nationwide Qualified Health Information Networks for a comprehensive view of other encounters, prescriptions, or specialists.

It's relevant in nearly every way when it comes to proactively monitoring and servicing the health and wellness of remote patients while tracking patients' progress relative to Care Plan targets for documented proof of improved outcomes, better ED utilization, reduction in 30-day readmissions, reduction in DRG denials, and higher patient satisfaction. Our platform is flexible and program-based, allowing your organization to tailor it to fit your patient demographic needs. You can start using your own nursing staff / Community Paramedics, or we can supply licensed nurses to service your patient population using our platform as the intelligence layer to augment telemedicine calls. We can also handle a hybrid model as your nurses gain comfort with the dashboard. We have a logistics center that will package up the devices and ship them out to enrolled patients while also handling Tier 1 Support calls. In addition, there is a self-help section for FAQ.
