Year 1 is entering a conversion phase rather than ending cleanly: the opportunity-discovery wave peaked in July 2026, and the operational task now is triage — reconciling 394 past-due records into a real disposition rather than a stale status label.

The Rural Health Transformation Program is also not a one-time grant — it is a five-year cooperative agreement between CMS and each state, structured into annual budget periods. Across 29 of 50 tracked states, source documents independently cite the same national checkpoints this fall to open Year 2. The tracked opportunity pipeline does not reflect that yet, which is exactly the gap this report exists to flag before it becomes a surprise.
