This Notice of Funding Opportunity (NOFO) solicits proposals from licensed Vermont nursing facilities to establish and expand in-state dialysis and ventilator services for long-term care residents, focused on rural access. Funded through Vermont’s Rural Health Transformation (RHT) program and the CMS grant award #RHTCMS332047-01-00, the initiative supports acquisition of specialized equipment and staff training (not facility renovations) with the goal to increase sustainable capacity for high-acuity care in nursing facilities. All dialysis proposals must include partnership with a qualified End Stage Renal Disease (ESRD) provider, and ventilator proposals must demonstrate partnership with an Acute Care or Critical Access Hospital. Grants are for 14 months (August 1, 2026–September 30, 2027) with required quarterly reporting and may be extended for up to four additional years with approval. Selection is based on alignment with the RHT program, proposed plan, organizational capacity, partnerships, and sustainability.
Applicants must be licensed Vermont nursing facilities in good standing with CMS, able and willing to submit accurate, complete, and timely quarterly progress and financial reports, and able to comply with State and Agency of Human Services subrecipient grant agreements.
