the program enables Texas to launch statewide efforts to improve rural Medicaid and hospital sustainability through initiatives focused on prevention and wellness, health IT, telehealth, workforce development, cybersecurity, and infrastructure enhancement for rural healthcare providers. The program's objectives include expanding rural care access, advancing technology and digital health, supporting provider sustainability, workforce recruitment and retention, and strengthening overall population health outcomes.
Eligible applicants are limited to the State of Texas, as the Texas Health & Human Services Commission is the official applicant and recipient. Sub-award opportunities may exist for rural health providers, hospitals, and community organizations as determined by state-administered processes.
Applications must be submitted electronically via the Grants.gov portal for the Rural Health Transformation Program (RHTP) by the designated state administering agency (i.e., Texas Health & Human Services Commission).
