Eligibility

Requirements Checklist

• Application requirements include a comprehensive statewide rural health transformation plan detailing at least three activities aligned with CMS strategic goals (preventive health, provider sustainability, workforce development, innovative care, or technology transformation), a detailed budget, letters of support from partnering rural healthcare entities, a population health impact statement, and an implementation and evaluation plan. Post-award, recipients must provide periodic program reporting to CMS and comply with all terms under the cooperative agreement.
