Tennessee RHT: Community Health Worker (CHW) Infrastructure Project

Eligibility

Any health care provider organization that serves TennCare members and employs or wishes to employ Community Health Workers (CHWs), including primary care and outpatient providers. Priority given to providers serving individuals residing in rural communities within Tennessee; providers whose primary service populations are non-rural remain fully eligible. Organizations must reach the minimum CHW/supervisor staffing requirement for their funding tier by the end of the grant period (do not need to already employ that number at time of application).

Requirements Checklist

• Applicants must submit a Grant Application (Attachment 1), Budget Narrative (Attachment 2), and Statement of Assurance (Attachment 3), as specified in the RFA. Organizations must implement the IMPaCT Community Health Worker model, participate in technical assistance activities, and pursue TNCHWA Program Accreditation. Year 2 renewal is contingent upon successful completion of Section A.6 deliverables, organizational capacity growth, maintaining or expanding CHW staffing to meet tier requirements, and State approval.
