Rural Health Transformation Fund Grant - Enhancing Primary Care Focused Data and Analytics

Summary

The Department of Vermont Health Access (DVHA) is soliciting applications for the Rural Health Transformation Fund Grant focused on enhancing statewide primary care data and analytics infrastructure. This grant seeks to expand access to electronic clinical quality measures (eCQM) calculation tools and community health team (CHT) tracking tools among Vermont Blueprint for Health recognized practices and administrative entities. Funding will support implementation, workflow redesign, training, and data reporting improvements for care coordination and performance improvement. All notifications and addendums are posted on the Vermont Business Registry website.

Eligibility

Eligible applicants include Vermont Blueprint for Health recognized primary care practices and administrative entities with Community Health Teams (CHTs). Federally Qualified Health Centers, Rural Health Clinics, Critical Access Hospital–affiliated primary care practices that are Blueprint-recognized may apply independently or via their Administrative Entity. Designated Agencies are not eligible.

Requirements Checklist

• Applicants must commit to (a) implementation and statewide reporting of at least one data/analytics tool (CHT encounter tracking or eCQMs), (b) demonstrate capacity and willingness to comply with performance reporting requirements, (c) submit numeric and narrative quarterly reports, (d) outline a sustainability plan beyond the grant period, (e) agree to State's terms and conditions, and (f) include all required attachments in application. Subcontractors must be disclosed with letters of commitment. Compliance includes monthly financial reports, quarterly programmatic reports, and audit/monitoring as specified. State and federal guidelines for allowable budget costs apply.
