Rural Behavioral Health Prevention Infrastructure Subgrants

Summary

the Rural Health Transformation Program (RHTP). The selected entity will provide objective assessment of program performance, financial, and outcome tracking; develop and implement a statewide data reporting process including subrecipients, contractors, and the Department; and create dashboards, reports, and presentation materials communicating RHTP progress and impact across Idaho communities. Key deliverables include an Evaluation Design and Logic Model (within 120 days), baseline evaluation reports, quarterly learning briefs, monthly data quality/system reviews, and an annual evaluation report. This work supports both required federal and state reporting and guides ongoing RHTP implementation and funding priorities.

Eligibility

Eligible applicants include nonprofit organizations, public or quasi-public entities, qualified private entities, and Idaho universities with demonstrated experience and success with projects of this magnitude.

Requirements Checklist

• Applicants must be able to meet RFP requirements and demonstrate a track record of independent evaluation, data systems design, and analytics projects at statewide or similar scale.
