the state's first Community Care Hub (CCH) — an operational backbone connecting local organizations and individuals to address health and social needs in rural communities, aligned with federal RHTP PATH initiative benchmarks. Up to $2,000,000 is available in Year 1, with one anticipated award; total program funding may reach up to $18,000,000 over 5 years, contingent on future CMS/HHS RHTP awards.
A legal entity with proven experience organizing and supporting a network of community-based organizations (CBOs) addressing health-related social needs. Must have the infrastructure to handle administrative functions on behalf of small CBOs, independent clinics, and health service providers, and demonstrate the ability to deploy and manage a regional approach tailored to rural Utah.
