Eligibility

Year 1: Rural hospitals in New York State are eligible. Years 2–5: Additional rural health care entities, including pharmacies, nursing homes/long-term care, school districts, and other partners may be eligible, subject to program guidance, eligibility, and funding availability.

Requirements Checklist
