Minnesota RHTP: Technical Assistance for Rural Health Care Entities in Support of Community & Clinical Linkages

Summary

SWIFT Event ID 2000018473. Provides up to $4,000,000 in technical assistance to rural health care entities in support of community and clinical linkages, funded under Minnesota's RHTP award. The Minnesota Department of Health seeks qualified vendors to provide statewide technical assistance to up to 125 rural health care organizations, including hospitals, clinics, Tribal Nations, FQHCs, and CCBHCs/CMHCs. Technical assistance may include consulting, training, strategic planning, and tools to support implementation of RHTP activities focused on chronic disease management, preventive care, expanding access to community-based services (including mobile units), and integrating frontline workforce models. Example activities include supporting evidence-based models for frontline workers (e.g., community paramedics, health workers, doulas) and local care delivery with mobile units. Tasks include developing and implementing TA plans, supporting culturally relevant guidelines, providing bimonthly (every other month) summaries to MDH of TA requests and solutions, annual progress reporting, collaborating with other TA vendors and MDH, developing scalable tools and resources, and participating in regular meetings with MDH. TA must be available virtually and in-person, at minimum once monthly per organization.

Eligibility

Vendors located in Minnesota are preferred, but vendors outside Minnesota are eligible. Vendors may be not-for-profit or for-profit entities. Demonstrated familiarity with Minnesota Rural Health Transformation Program, demonstrated experience working with rural hospitals, Tribal Nations, FQHCs, CCBHCs/CMHCs, and capacity to provide coordinated technical assistance statewide are required. Subcontracting is permitted, must be detailed in proposal.
