Louisiana RHTP: Regional Care Conveners and Navigation Networks Program

Summary

The Louisiana Department of Health (LDH), through the Office of Rural Health Transformation and Sustainability, is offering funding under the Regional Care Conveners and Navigation Networks Program to develop regional care conveners and networks coordinating physical, behavioral, and social service providers (including community and school-based navigators) for rural populations. Funding prioritizes evidence-based convener models with measurable outcomes that align with RHTP strategies and long-term sustainability. Core functions to be supported include network orchestration, data integration and reporting, population engagement, and program oversight and accountability. Awards will be made in the form of a Cooperative Endeavor Agreement (CEA), with a mandatory 10% cost share/match contribution. Eligible applicants must be LDH-defined rural organizations able to document financial, infrastructure, or access-related need, and must submit a sustainability plan as part of their proposal. Award size ranges from $500,000 to $2,000,000 annually. Indirect costs are capped at 1.5% of direct costs. Reimbursement is made for eligible expenses incurred; construction/renovation costs may not be grant-funded but may count toward match. Applications will be reviewed and scored against criteria including documented need, projected impact, feasibility, sustainability, and cost effectiveness.

Eligibility

Applicants must: (1) be organizations qualifying as rural according to the Louisiana Rural Health Transformation Program definition and methodology; (2) demonstrate financial, infrastructure, or access-related need to rural patients through a documented assessment; and (3) be able to execute a Cooperative Endeavor Agreement with LDH. Both nonprofit and for-profit entities may apply if they meet the rural requirement.
