Implementing Healthcare Organizations selected in Year 2

Seven healthcare organizations will be funded to hire CHWs and implement Food Is Medicine interventions in rural Kansas. Awardees will collaborate with KDHE and participate in CHW/AFIM training, workflow integration, and program evaluation.

Implementing Healthcare Organizations selected in Year 1

These organizations will continue implementing Community Health Workers (CHWs) into clinical workflows and will work to implement Accountable Food Is Medicine (AFIM) based on guidance and direction provided by KDHE. Participating organizations will undergo CHW/AFIM training and will partner with local agencies to sustain and evaluate program implementation.

UKHS Care Collaborative Association
