Requirements Checklist

• Submit a complete application package including Cover Page, Project Narrative, Work Plan, Budget & Budget Justification, Organizational Capacity Statement, Letters of Support/MOUs (if applicable), proof of SAM.gov registration, current Indirect Cost Rate Agreement, Pre-Award Financial Questionnaire, most recent Single Audit (if applicable), Signed Certifications & Assurances, and Assurance of Non-Duplication.
• Submit the Non-Duplication Assessment (Appendix H).
• Project Narrative must address Statement of Need, Goals & Objectives (SMART), Approach & Methods (evidence-based), Target Population & Engagement, Partnerships & Collaboration, Evaluation Plan, and Sustainability.
• Comply with all federal and state programmatic, financial, and reporting requirements.
• Participate in required monitoring, data collection, and evaluation activities.

• Project Narrative (10 pages max)
• Work Plan with Milestones & Timeline (prioritize Year 1; option to submit multi-year plan)
• Budget & Budget Justification (must use provided Excel template; detailed narrative required)
• Organizational Capacity Statement
• Letters of Support/MOUs (if applicable)
• Proof of SAM.gov registration
• Current Indirect Cost Rate Agreement (if applicable)
• Pre-Award Financial Questionnaire (template provided, Appendix F)
• Most Recent Single Audit (if applicable)
• Signed Certifications & Assurances
• Assurance of Non-Duplication (template provided, Appendix H)
• Sustainability Plan (describe continuation beyond grant period)
