Summary

This solicitation seeks applications from eligible organizations to implement evidence-based behavioral health prevention programs throughout rural and underserved communities in Idaho. Projects should expand access to prevention services, promote protective factors, reduce behavioral health risk, support early identification and intervention, and strengthen the behavioral health continuum of care. Funding supports new or enhanced prevention services, workforce development, and innovative service delivery models to improve community health. All activities must comply with relevant state statutes, including the Idaho Parental Rights Act. Administrative/indirect costs are limited to 10% of Modified Total Direct Costs (MTDC), per CMS requirements. Awards will be made based on the criteria outlined herein, and the Department reserves the right to fund applications out of rank order to ensure equitable geographic distribution and population reach.

Eligibility

Eligible applicant entities include rural healthcare facilities, schools, community organizations (including faith-based entities), institutions of higher education located in Idaho, nonprofit organizations with 501(c)(3) status, state/local government agencies, tribal organizations, healthcare associations and professional societies, healthcare quality improvement organizations/collaboratives, hospitals and health systems located in Idaho, Federally Qualified Health Centers (FQHCs) and FQHC Look-alikes, and healthcare providers in rural Idaho.

Requirements Checklist

Submit a complete application package including Cover Page, Project Narrative, Work Plan, Budget & Budget Justification, Organizational Capacity Statement, Letters of Support/MOUs (if applicable), proof of SAM.gov registration, current Indirect Cost Rate Agreement, Pre-Award Financial Questionnaire, most recent Single Audit (if applicable), Signed Certifications & Assurances, and Assurance of Non-Duplication.
Submit the Non-Duplication Assessment (Appendix H).
Project Narrative must address Statement of Need, Goals & Objectives (SMART), Approach & Methods (evidence-based), Target Population & Engagement, Partnerships & Collaboration, Evaluation Plan, and Sustainability.
Comply with all federal and state programmatic, financial, and reporting requirements.
Participate in required monitoring, data collection, and evaluation activities.
