Georgia RHTP: Consumer Engagement Enhancements

Summary

Extends the State Health Benefit Plan's consumer wellness platform—including an AI-generated Geospatial Health Risk Profile, diet and wellness coaching tools, and integrated behavioral health components—to the AHEAD-model population in 126 HRSA-designated rural counties. The initiative is part of a broader technology-driven effort to improve care coordination, patient engagement, and health outcomes, supported by partnerships with academic institutions, healthcare providers, and technology firms. Expansion to rural health clinics and FQHCs is planned in Years 2-5 as budget permits.

Eligibility

Eligible applicants include Georgia-based healthcare providers, rural health clinics, federally qualified health centers (FQHCs), academic institutions, and technology firms partnering with healthcare entities. Serving the AHEAD-model population in 126 HRSA-designated rural counties is required.
