Expanding Care: Doulas Program

Summary

The program funds up to $1,000,000 per component per applicant, supporting training for doulas, parent education/awareness, provider partnerships, and network-building. Priority is given to experienced rural-serving organizations meeting specific licensure, federal registration, and insurance requirements. OSDH reimburses eligible costs; detailed budgeting, required forms, and adherence to federal and state compliance are mandatory.

Eligibility

Applicants must demonstrate experience with doula programming or training, and have a track record of partnerships in rural communities. Entities can be in- or out-of-state (preference to Oklahoma-based). Applicants must deliver direct services in eligible Oklahoma rural communities (pop. under 55,000; see Attachment A). Applicants must: maintain required licensure/certification for all services, have an active UEI and SAM.gov registration, not be suspended or debarred, carry required insurance (see Attachment D), and have or be applying for an Oklahoma Supplier ID. Fiscal sponsorship is permitted if documentation is included.

• Each application must be submitted per program component. Required materials: completed Organizational Information Form, Project Plan (Attachment F), Workplan (Attachment G), Budget Forms (Attachment C), staff list with certifications, Grants Management Packet (Attachment D incl. SRQ, COI, audit/financial statement, NICRA if applicable), letters of support, proof of nonprofit status (if applicable), signed OSDH Terms & Conditions. All files must be uploaded via the online portal; ensure all templates provided are used. Applications must attest to review of subrecipient responsibilities and certify that awarded funds do not supplant existing funding.
