DOH Rural Health Transformation Grant & Project Management

Summary

The Rural Health Transformation (RHT) initiative is a statewide effort focused on strengthening rural healthcare systems and improving access to quality care across South Dakota. Through strategic investments, innovative care models, and cross-agency collaboration, Rural Health Transformation is designed to build a more resilient, connected, and sustainable rural health system for communities across the state. The State of South Dakota Department of Health (DOH) is seeking a partner agency to provide two qualified individuals to support the South Dakota Department of Health (DOH) with the statewide Rural Health Transformation Project. The individuals must be able to support the financial and administrative functions of the multi-year grant full-time. At the highest level, the day-to-day work will include, but is not limited to, administrative support of contract management, subrecipient monitoring, program operations, project management, strategic oversight, data, evaluation, and report collection, etc. The partner will provide experts on their team to work directly with South Dakota's RHT Principal Investigator Project Director and Financial Grants Specialist for the state's Rural Health Transformation Project.

Eligibility

Eligible applicants are public or private organizations licensed and authorized to conduct business in the State of South Dakota. Applicants must demonstrate adequate organizational capacity and prior experience managing similar healthcare transformation or public health initiatives. Individual consultants or sole proprietors are not eligible. See RFP Section 2.2 for details.
