Connecticut RHTP – Health Care Coordination and Remote Patient Monitoring Using Artificial Intelligence (NOFO #26OHS001)

Requirements Checklist

• Applicants must submit: 1) a signed OHS grant application form; 2) a detailed project narrative (≤15 pages) describing AI-enabled care coordination and RPM activities and measurable goals for Year 1; 3) a summary work plan with milestones; 4) organizational background and capacity (max 4 pages); 5) a detailed line-item budget and justification (using OHS Budget Workbook); 6) letters of commitment from key partners, as applicable; 7) pre-award risk documentation, financials, and compliance certifications; 8) State Agency Vendor Form (SP-26NB), IRS Form W-9, current fiscal and tax filings, board-approved budgets, and budget vs actuals for the current and previous years. Projects must demonstrate workflow integration with Connie (CT HIE). All expenditures must be incurred in the approved budget year; no carryover. Projects must be new or meaningfully expanded and undergo a pre-award risk and compliance review.
