Sent to vendors scoring under 4.0 on RHTP Fitness on July 9, 2026. Rural Care Journey introduced the RHTP Fitness Score, with support from Survey Health, to help states, providers, and partners quickly understand each vendor's readiness and fit for Rural Health Transformation Program opportunities.
Many vendor profiles were still missing important information at the time, which can cause scorecards to appear unintentionally low. Reviewing and updating these five areas is the most direct way to improve the score:
- 1Rural Fit — the rural settings you support: public health departments, rural hospitals, FQHCs, EMS, clinics, schools, or community-based organizations.
- 2RHTP Alignment — the RHTP priorities your solution supports: care coordination, telehealth, workforce training, behavioral health, EMS, cybersecurity, chronic disease, or data/reporting.
- 3Submission Quality — clear language about what you do, who you serve, and how your solution supports rural transformation.
- 4Problem-Solution Fit — the specific problems your organization helps solve. Missing problem statements can significantly lower this part of the score.
- 5Credibility & Evidence — case studies, outcomes, references, regulatory signals, implementation experience, or other evidence of readiness.
