How RHTP Funding Flows from CMS to Rural Communities

Updated October 12, 2026

RHTP funding is a federal-to-state implementation model. CMS establishes the federal program and approves state plans; states then decide how to translate those plans into programs that rural organizations, providers, communities, and vendors can use. The result is not one uniform grant calendar. It is a connected set of state solicitations, direct awards, contracts, technical-assistance programs, and implementation projects.

The five practical stages

  1. 1CMS establishes the federal framework, requirements, reporting expectations, and state funding allocations.
  2. 2Each state develops and implements a strategy around its rural access, workforce, technology, care-delivery, and sustainability priorities.
  3. 3State agencies publish RFAs, RFPs, NOFOs, grants, RFIs, or other notices — sometimes through procurement portals and sometimes through agency pages.
  4. 4Awards and contracts move funding to providers, local governments, universities, community organizations, and implementation vendors.
  5. 5Awardees deliver projects, report outcomes, and create the next evidence that informs amendments, future rounds, and sustainability decisions.

Why the path looks different in every state

States have different rural geographies, procurement rules, administrative structures, and delivery-system priorities. One state may publish a competitive RFP for a statewide health information exchange; another may issue small provider grants, a workforce RFA, a technology challenge, or a direct award. A notice can also be open to vendors, restricted to eligible rural entities, or non-competitive. Always read the source notice and attachments before treating a record as an application opportunity.

How to follow the money in RCJ

  • Start with the state page to understand the strategy, active programs, documents, and award activity.
  • Use the opportunity page for eligibility, deadlines, application instructions, linked files, amendments, and source evidence.
  • Check award and healthcare-entity records to see who received funding and what implementation capability is appearing in the market.
  • Use reports and guides for cross-state patterns, while treating the official issuer source as the final authority for legal and program decisions.
