# West Virginia Rural Health Transformation Program Start-Up Funding for SUD Evidence-Based Practices (2026)

The West Virginia Rural Health Transformation Program (RHTP), administered by the West Virginia Department of Health under the Smart Care Catalyst initiative, is soliciting applications for Year 1 start-up funds and provider incentives to implement or expand evidence-based practices in substance use disorder (SUD) prevention, treatment, and/or recovery. Required strategies prioritize upstream, root-cause-focused solutions over episodic, downstream care and include explicit integration with the State’s RHTP data spine, use of non-duplicative and non-billable activities, robust operational and outcome metrics tracking, and ongoing participation in State-led VBC design workgroups. Funds are awarded and released in milestone-based tranches: (1) program design/operational readiness, (2) initial implementation/service delivery, and (3) achievement of performance thresholds and sustainability. The goal is to expand rural/underserved service capacity, reduce avoidable emergency/crisis-related system utilization, improve population-level SUD metrics, facilitate workforce reintegration, and strengthen long-term system transformation consistent with State MAHA principles and the Governor’s Four Pillars. All grantee data, reports, and workflows become State assets for analytics, oversight, and continuity. This one-year funding opportunity, totaling $6,000,000, ends July 31, 2027; continued funding is contingent on State/CMS appropriations and demonstrated grantee outcomes, as defined by RHTP and CMS/HHS requirements.

## Eligibility

Eligible applicants are provider organizations, behavioral health and SUD treatment providers, Federally Qualified Health Centers (FQHCs), hospitals, community-based organizations, and cross-sector partnerships with demonstrated capability to implement or expand SUD-focused interventions aligned to successful participation in future value-based care (VBC) models.

## Requirements

Applications must be submitted via the West Virginia OASIS Vendor Self-Serve Portal using Solicitation No: GFO-0506-2700000000036-1. Required materials: Project Narrative; Detailed Line-Item Budget and Budget Narrative per RHTP DLIB Budget Instructions; Grantee Contact Information Form. Proposals must specify the evidence-based intervention/model, target population/geography, and demonstrate that activities are net-new or represent meaningful expansion beyond current reimbursable services. Applicants must evidence operational readiness, demonstrate capacity for near-term measurable improvement, integrate all operational/program/outcome data with the RHTP data spine (per State technical, governance, and CMS/HHS standards), and maintain HIPAA/42 CFR Part 2 compliance. Participation in recurring State VBC workgroups, adherence to RHTP/CMS/HHS milestone, reporting and compliance requirements, and explicit non-duplication of services are mandatory. Clearly defined operational goals, partnership commitments, and readiness for milestone-based implementation/payment must be demonstrated.
