West Virginia Newborn Hearing Screening Project

Summary

the state to help guide them through the EHDI system and to offer emotional support as they process their child's diagnosis of hearing loss. The grantee must provide timely outreach, parent education, and required data and financial reporting in compliance with NHS and Early Hearing Detection and Intervention guidelines. Funding is available for April 1, 2025, through March 31, 2026. Services help meet objectives including initiation of follow-up services within two business days of referral, support for DHH families, timely and accurate data maintenance, and collaboration with the NHS Project Director.

Requirements Checklist

• Applicants must: provide a proposal narrative (editable text file) with introduction, coverage area, objectives, activities, outputs (performance measures), and timeframe; submit a detailed line-item budget (DLIB) with justification narrative; submit a signed Sub-recipient Information Form (DHHR Finance A-1000) with matching address/zip as listed in CCR; provide a Grantee Contact List with at least two staff covering all reporting and grant components; maintain current SAM.gov and WV Secretary of State registration; submit all required documents through BOTH the WVOASIS VSS Portal and via email to DHHRBPHGrants@wv.gov. If requesting startup funds, submit a separate startup budget and justification for non-recurring costs (e.g., training, equipment, fees), in accordance with Section Three of the AFA.
