## Eligibility

Applicants must be physically located in, or provide direct services to, rural Virginia communities as defined under Virginia's RHTP (FORHP baseline definition). Projects must target residents of: Bland, Buchanan, Carroll, Dickenson, Grayson, Lee, Russell, Smyth, Tazewell, Wise, and Wythe counties; cities of Galax and Norton.

## Requirements

Required: 1) Complete and submit Narrative Response (10 pages max), including Project Description, Target Population, Implementation Plan, and Sustainability Approach, as set forth in the RFA section IV; 2) Complete Budget Template (Attachment B); 3) Complete Applicant Certification & Assurances (Attachment C); 4) Letters of Commitment from partner organizations if relevant. All templates and forms must be included and completed as specified in the RFA. Proposals must demonstrate the ability to deliver qualified mobile or hybrid care models and must address Community Need, Project Work Plan, Organizational Capacity, Evaluation Plan, and Budget Justification as per review criteria.
